All dental services offer by an authorized provider in a participating network are eligible for savings ranging from 15% to 50% under the plan per visit . No dental services offered by a participating provider for which no other discount is available are excluded.
When visiting a participating dental office, present you membership card prior to treatment to be assured that the proper discount is applied. Payment for services is due at the time they are rendered. Provider lists and fees may change at any time.
